Special Information for Victims of Human Trafficking

When submitting such a request, please ensure you include appropriate proof of identification, victim determination documentation (i.e., documentation that demonstrates you are a victim of trafficking) and identification of the specific adverse information you would like to block or dispute.

Examples of acceptable proof of identity include:

  • Government issued ID

  • Birth certificate
  • Social Security card
  • Utility bills (showing name and address)
